It is dated September 20, 2002 (which is shortly after them being taken..sept. 2nd 2002)
To whom it may concern:
I am writing concerning the medical care and treatment initiated by Jennelle for her children, Haven, Keghan, Zachary and Lily.
My primary contact has been with Zachary since 9 months of age. Though his vaccinations were initially delinquent, Jennelle complied with the recommended requirements to up-date his required immunizations. During this period, Zachary's growth and development were within normal limits. Jennelle also sought medical attention for Zachary appropriately and followed the prescribed medical advice.
I have taken care of Lily since birth. Jennelle primarily breast-fed Lily for the first 6 weeks of her life. She also scheduled and kept Lily's routine well-child appointments and immunizations. As with Zachary she sought medical attention when Lily was ill.
My care of Keghan and Haven was limited to one well-child exam each in August of 2001. Both children were up-to-date with their immunizations at that time. Their growth and development were age-appropriate. Keghan's exam was only significant for dental caries and a dental exam was recommended at that time.
In general, Jennelle has complied with the necessary preventive and urgent medical care for her children.
